About this form

  • If you are a registered Minnesota voter, use this form to request to be a permanent absentee voter.
  • To complete this form, you will need to provide:
    • Your ID numbers:
      • A Minnesota driver's license or ID card
      • The last 4 digits of Social Security number
    • An email address
  • If you do not have or do not want to send this information online, you can fill out a paper version of this form and return it to your local elections office. Absentee ballot application (PDF)
  • You have 10 minutes to complete each step of this application. If you do not finish a step within that time, you will start over at Step 1.
  • If you are a participant in Minnesota's Safe at Home Address Confidentiality Program, do not use this form. Contact Safe at Home at 1-866-723-3035.

  • Letters A to Z, uppercase or lowercase. Do not use letters with accents or diacritics (such as E with an accute accent or N with a tilde) or non-English characters (such as Cyrillic letters or Chinese characters).

I cannot find my street

If you do not see your street listed:

  • Search for variations of your street's name or spelling (for example, Hwy 7 instead of Broadway).
  • Verify that you either entered the correct ZIP Code or selected the correct county, city and ZIP Code.
  • If you entered anything incorrectly, change your entries or selections.
If your registration includes an apartment or other unit number, you must include it in your search.
