About this form

If you are a registered Minnesota voter, use this form to request to be a permanent absentee voter.
- As a permanent absentee voter, an absentee ballot will be sent to your residence address for each election you are eligible to vote in.
- If you require a ballot to be sent to an address different from your residence, do not use this form. An absentee ballot can be requested through the Minnesota Absentee Ballot Request.
To complete this form, you will need to provide:
- Your ID numbers:
  - A Minnesota driver's license or ID card
  - The last 4 digits of Social Security number
- An email address

You have 10 minutes to complete each step of this application. If you do not finish a step within that time, you will start over at Step 1.
