Use this form to request absentee ballots for federal and county elections.
- For city, town or school district elections, send a completed absentee ballot application to your local city, town or school clerk. Absentee ballot application (PDF)
- To have a ballot mailed to your residence for all elections, update your voter registration and request to become a permanent absentee voter.
To complete this form, you will need to provide:
- Your ID number: a Minnesota driver's license or ID card or last 4 digits of Social Security number
- An email address

You have 10 minutes to complete each step of this application. If you do not finish a step within that time, you will start over at Step 1.
